My mistake on the V2. I went off an old sheet of instructions that had the EX and PS columns in the wrong order. So, basically all the EX and PS for the skaters were off. I noticed during a game the other team took a penalty, then won the face-off and scored short handed a few seconds after the penalty. It looks like the face-off was never moved to the defensive end of the team taking the penalty.
