Substitution on PK?

Last game one of my forwards (Galchenyuk) played :56 seconds on the PK, however he is not listed on any PK lines and he's not listed as an extra forward for the PK. So I went looking through the play-by-play and this is what I found...

End Of Time 18:
Minor Penalty to Erik Karlsson for Hooking. Penalty Kill Forward Lineup #1 - Trevor Lewis, Alex Galchenyuk are on ice for Colorado Avalanche. Penalty Kill Defense Lineup #1 - Drew Doughty, Luke Schenn are on ice for Colorado Avalanche. John Tavares wins face-off versus Trevor Lewis in Colorado Avalanche zone. Pass to Max Pacioretty. Pass to Jakub Voracek. Pass to John Tavares. Pass to Ryan Suter. Pass to Jakub Voracek. Pass to Max Pacioretty. Pass to John Tavares. Shot by John Tavares. Stopped by Viktor Fasth with a rebound. Free Puck Retrieved by Ryan Suter for San Jose Sharks. Shot by Ryan Suter. Stopped by Viktor Fasth without a rebound. Minor Penalty to Viktor Fasth for Delaying the game.

Time 19: PowerPlay Forward Lineup #2 - Derek Stepan, Johnny Gaudreau, Steven Stamkos are on ice for San Jose Sharks. PowerPlay Defense Lineup #2 - Erik Johnson, Anton Stralman are on ice for San Jose Sharks. Penalty Kill Forward 3vs5 Lineup #2 - Brock Nelson are on ice for Colorado Avalanche. Penalty Kill 3vs5 Defense Lineup #1 - Seth Jones, Alex Galchenyuk are on ice for Colorado Avalanche.

In the first play-by-play paragraph Galchenyuk is put out on PK1. Then at Time 19 there is another penalty and this time Galchenyuk gets put on defense for 3vs5 PK1.

Galchenyuk shouldn't be out there for the PK at all and certainly not on defense. So just wondering why this might have happened?

At some point, if all your players are too tired to play, anyone will be send on the ice.

I guess that makes sense but obviously I'd rather see a D than a F on defense for the PK :?

Any reasoning for this....

Normal Forward Lineup #3 - Nathan MacKinnon, Jonathan Huberdeau, Josh Jooris are on ice for Colorado Avalanche. Normal Defense Lineup #4 - Alex Galchenyuk, Anders Lee are on ice for Colorado Avalanche.

Two forwards sent in for D4??

I have a look at it and the only assumption I can make is that all your defenseman were too tired to be send on ice. You only have 2 face-offs in 9 minutes and without breaking to recover, I assume all the defenseman got tired.
