A few questions:
1) Where do coaches salaries come from? Are they subtracted in the offseason?
2) What does the effect of rivalries mean when it says "intensity levels of the players increases". What exactly does that change?
3) Is there a benefit to taking penalties in the game? Does it increase "intensity" or morale or anything?

1) From the rating, I would assume from the NHL. Subtracted????
2) Team get more physical = More Hits, More Penalty, More Fight
3) Euh. In there a benefit in taking penalies in a real NHL games?

At what point do all these expenses come out of the bank account? (ex; coach salary is taken out after you play the last game of the season)

Each days of your scheduled, players and coaches are paid a % of their salaries.
The % is calculated by (100 / # of Days in your Schedule)

The default starting money is $10 000 000. I know it's up to me to change it but I'm guessing that amount implies that amount is enough to start off with? Then how can teams pay up to $60 000 000 in salaries with just $10 000 000 to start with?

It's all related to your arena's capacity, the price of tickets, and the average price of tickets. Your star power players (SP) also have a small influence.
So, basically, if your calendar runs 200 days and you have a payroll of 60M and a coach of 1M, and an 82-game calendar...
that means each day...
You will pay 305,000$. (61M / 200 * 1day)
You'll get 41 home game, so say you won't make the playoffs, it means you need to earn 51M in 41 games,
or 1,243,902$ per game.
You sell tickets according to your ticket price vs. league average ticket price. So if you're much higher than your league average (ex : you sell tickets at 200$ but the league's average is at 100$), then you will have little attendence. On the other hand, if your league's average is at 200$ and yours are at 100$, you have a much better chance to sell out.
For realism's sake, if you are in charge of your league, I recommand setting minimal and maximal ticket prices for your GMs.
